About the Program
Main Job Purpose
Provide comprehensive, nurse-led healthcare to address the unique challenges faced by young women regarding HIV/AIDS, sexual and reproductive health, and overall wellbeing. Deliver a package of health interventions informed by regular needs assessments, including: HIV testing services (including self-screening), HIV prevention and PrEP provision, ARV treatment, case management, viral load monitoring, TB and STI screening, sexual and reproductive health services, basic post-violence care, HPV screening and referral, and prevention of vertical transmission in pregnant women living with HIV. Establish effective linkages to care channels, manage healthcare needs, and ensure treatment completion for adolescents and young people.
Key Performance Areas
ART Initiation, TB, Hep B & C, and NCD Linkage to Care
- Initiate all diagnosed HIV-positive clients on ART daily.
- Build relationships and establish linkages with healthcare facilities across regions.
- Utilize local health facilities for clinical supplies and client treatment.
- Ensure clients testing positive for HIV, TB, Hep B & C, and other NCDs (Diabetes, Hypertension) are linked to care, monitored, and complete treatment.
- Manage assigned cohorts to achieve viral suppression and treatment completion.
- Submit weekly, monthly, and quarterly case management and related reports.
- Conduct follow-ups for assigned cohorts and submit monthly cohort management reports.
- Maintain accurate records of clinical materials and ART medication, including daily reconciliations.
- Provide regular reports of ART initiations to the project manager and relevant stakeholders.
Outreach Clinical Services
- Collaborate with psychosocial counsellors to conduct adherence support clubs and related groups.
- Assess client needs for HIV prevention, care, and support, and develop individualized plans with the Site Coordinator, Linkage Nurse, HTS Counsellors, and Linkage Officers.
- Provide outreach clinical services including HTS, STI and TB screening, sputum/X-ray collection, STI treatment, wound care, ART, PEP, PrEP, non-communicable disease screening, cervical cancer screening, family planning services, and referrals for specialist care.
- Collect sputum and ensure treatment initiation and completion for clients testing positive for HIV or TB.
- Ensure all outreach services adhere to National Department of Health guidelines and meet or exceed project targets.
- Support ART adherence and monitor viral suppression for positive clients.
Project Support and Administration
- Conduct daily reviews of HTS and ART registers.
- Participate in weekly project meetings and stakeholder engagements.
- Maintain up-to-date clinical follow-up registers and attend to referrals from team members.
- Provide oversight to HTS counsellors, linkage officers, and social auxiliary workers.
- Update clinical records and provide portfolio statistics.
- Manage drug supply and HIV RDTs in line with Good Pharmacy Practice (GPP).
- Monitor HTS data, track referrals, and validate successful linkages.
- Familiarize with Anova policies and attend relevant meetings, trainings, and workshops as required.
Organizational Support
- Counsel and train staff on clinical ethical conduct.
- Stay current with clinical developments, guidelines, and training.
- Assist in setting goals, objectives, and interventions for quality service delivery.
- Ensure medical equipment is functional and maintain adequate clinical supplies and stationery.
- Adhere to Health and Safety and Infection Control Policies.
- Implement and support quality assurance programs such as Ideal Clinic Realisation and Maintenance (ICRM) and National Core Standards (NCS).
- Conduct assessments and implement Rapid Test Continuous Quality Improvement, including Independent Quality Control and Proficiency Testing for HIV RDTs.
- Attend meetings, briefings, and staff sessions as directed.
- Ensure adherence to Anova vehicle management policy when traveling to work sites.
- Perform additional responsibilities as delegated by management.
